Your privacy is important to us.

All information pertaining to you and your use of the Internet charting service is held in an on-line database. Access to your records in the database is protected by your password.

The information that is held consists of
1. The geographic location and internet service provider's address that is submitted by you in order to gain a user ID and password;
2. Your privacy options and service agreement;
4. Your fertility observations, and messages interchanged with the service.

The key threats to the privacy of this information are
1. the security threats to information on the Internet that is not encrypted, and only protected by password
2. the confidentiality of your userID and password.

For this reason there is no personal identifying information recorded about you. The only link between you and the information recorded about you is your userID.

You are free at any time to reverse your enrolment (drop unit). If you submit this request your fertility records will be removed from the on-line database.

You may also de-register, in which case your registration information will be removed from the on-line database and your userID and password will no longer be recognised.

However in both these cases your records may persist in database archives and it will take some time for them to be purged. During this period you cannot have access to the records.

Depending on the response that you submit to the following questions your fertility records may be used as described below. You must respond to question 1 in the affirmative in order to use the service.

1. I authorise recording of such information as is necessary for the adequate provision of the service that I have sought. YES/NO
2. I authorise the teacher to use my de-identified data for future research or statistical collection purposes or to use my de-identified fertility chart for training purposes. YES/NO
